Criminal Records Check (CRC)
Sherwood Park Ball Hockey requires Coaches to provide documentation on background searches.

If you have a CRC from another sport SPBH we will accept the original or a copy dated within the past year.
Note:
- CRC application to be completed at Police/RCMP Detachment (where you reside)
- Official SPBH Letter - click here (print letter to accompany application)
- You will need 2 pieces of ID (at least one photo)
- Strathcona Residents - No Fee (you MUST take official SPBH letter with you)
- Strathcona Non-Residents - must be completed in jurisdiction where volunteer resides
Please ensure your CRCs are completed/handed in by: April 30
Coaches will NOT be allowed on the bench after this date if CRC is not received.
