Criminal Record check (CRC)

Sherwood Park Ball Hockey requires Coaches to provide documentation on background searches.

If you have a CRC from another sport, SPBH will accept the original or a copy dated within the past year.

Note:

• CRC application to be completed at Police/RCMP Detachment (where you reside)
• Official SPBH Letter - click here (print letter to accompany application)
• You will need 2 pieces of ID (at least one photo)
• Strathcona Residents - No Fee (you MUST take official SPBH letter with you)
• Strathcona Non-Residents - must be completed in jurisdiction where volunteer resides​​​​​​​

Please ensure your CRCs are completed/handed in by: April 26

• Drop off in SPBH box (RRR lobby) or Scan to spbh@parkballhockey.ca

Coaches will NOT be allowed on the bench after this date if CRC is not received.
